Club Annual Achievement Form
This form is to be completed at the end of each Key Club Year. By completing a specific point value, determination is made as to whether the club qualifies for the Distinguished Club Status or the Diamond Level Distinguished Club Status. A banner patch is received by the clubs meeting the point value. This report can be turned in at the start of our District Leadership Conference in March; or can be sent via email or mail to the District Administrator.
